Position Overview

As a Healthcare Social Worker, you will play a crucial role in providing support and assistance to individuals and families facing medical challenges. Your responsibilities will include conducting assessments, developing care plans, coordinating resources, and offering emotional support to help patients navigate the complex healthcare system and address social and emotional needs.

Responsibilities

• Conduct comprehensive assessments of patients' social, emotional, and environmental needs.

• Collaborate with healthcare professionals to develop and implement individualised care plans.

• Provide counselling and emotional support to patients and their families, addressing coping mechanisms and adjustment to illness or treatment.

• Assist patients in accessing community resources, such as financial assistance programs, support groups, and home care services.

• Advocate for patients' rights and collaborate with interdisciplinary teams to ensure continuity of care.

• Coordinate and facilitate discharge planning, including arranging post-hospital care and necessary follow-up appointments.

• Provide education and guidance to patients and families regarding healthcare options, advance directives, and available support services.

• Maintain accurate and confidential documentation of patient interactions and progress.

Qualifications

• Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.

• Current and valid social work license or certification, as required by the relevant jurisdiction.

• Previous experience in a healthcare or medical setting is preferred.

• Knowledge of social work principles, practices, and ethics.

• Excellent communication and interpersonal skills.

• Strong assessment and problem-solving abilities.
